Requirements
- Completed Application Form
- Taxpayer Identification Number (TIN)
- Valid identification (Passport or National ID)
- A valid Trade Licence
- Licensed Pharmacist Certificate
- The applicant must have a Business Registration from Deeds Registry, Companies Act
- Drug Certificate from the Poison Board, Ministry of Health
- Sanitary Certificate from Mayor & City Council or National Democratic Council
- An approved plan and letter from the Ministry of Housing
- Safety Certificate from the Guyana Fire Services

- An approved plan of the premises, stamped and signed by the Central Housing and Planning Authority and Neighbourhood Democratic Council or Mayor and Town Council or Regional Democratic Council, Guyana Fire Service and Sanitary Department.
- Approval from Toshaos for areas controlled by same.
- Copy of Transport.
- A Tenancy Agreement, if premises is leased or rented, or an Affidavit of Consent from the owner(s) of the property or their duly authorised representative.
- A Fire Certificate from Guyana Fire Service.
- Sanitary Certificate from the Mayor & City Council or Regional Democratic Council’s Sanitary Department
- Updated Business Registration from Deeds Registry, Companies Act or Articles of Incorporation
- Drug Certificate from the Poison Control Board, Ministry of Health (pharmacies, supermarkets and shops interested in selling Over -The-Counter (OTC) drugs)
- Licensed Pharmacist Certificate for Pharmacy or business selling prescription drugs.
Shop – Drug 1st & 2nd Class Licence
